REGISTRATION & PAYMENT INSTRUCTIONS:

• Payment must be made in U.S. funds.
• Make check payable to Foundation of FTA.
• Only individuals 16 yrs. of age or older who are registered and badged may attend technical sessions & INFO*FLEX.

CANCELLATION POLICY:

• All cancellations must be submitted to FFTA in writing.
• Cancellations for Full Forum registrations prior to March 14, 2008 will incur a $100.00 processing fee. 
• Cancellations for ALL registrations & golf received after March 14, 2008 will not be refunded.
• No refunds will be issued for no-shows.
• Substitutions welcome provided they are submitted in writing.
• Because the Golf Tournament is a charitable event, no refunds will be given should the event be rained out.
